Bypass Surgery

Depending on the degree of arterial stenosis, your doctor may recommend a coronary artery bypass operation. Coronary artery bypass surgery re-supplies blood to the heart through a different path created with the help of a graft. In the event that there are more than one constricted artery, more than one bypass may be required. The graft to be used for the bypass is taken from the inner wall of the chest, arm or leg of the patient and connected to the constricted coronary artery. The most commonly used grafts are internal thoracic arteries from the chest wall, radial artery from the arms, and saphenous vein from the legs.

Minimally Invasive Surgery

Minimally invasive technique is a method in which the open-heart surgery is performed deploying special endoscopic devices through a small incision. During the operation, all surgical instruments are under the control of the surgeon and an endoscopic camera can be used to view the surgery area. 

Minimally invasive technique can be used for coronary bypass, valve repair, valve replacement, repair of congenital defects of the heart like ASD and treatment of heart rhythm problems. Compared to open surgery, minimally invasive surgery offers small incisions and small scars, mobility is higher, and scar healing is much faster. Following surgery, patients return to work and resume active life much earlier. Most patients also benefit from

advantages such as less pain, lower risk of infection, lower risk of bleeding, and lower need for blood transfusion. However, not all patients can be suitable for minimally invasive surgery. Your doctor will decide whether you are eligible after assessing your condition and your test results.

 

Transcatheter Aortic Valve Implantation (TAVI)

TAVI involves the implantation of an aortic valve to the heart using the catheter method without open surgery. In TAVI method, a biological valve is placed in a stent jacket, when opened, is firmly placed on the valve implant area.

Two different techniques can be used in this method. In the first technique, similar to  angiography, the valve is deployed through the inguinal vessels to the heart with the help of a catheter. When in place, the stent mechanism is opened. The second technique is used when there is a stenosis in the inguinal vessels or the abdominal vessels that will be used to access the heart.

Then a small incision of 4 to 5 cm is made in the front wall of the sternum to access the apex of the heart. The valve is placed with the help of a catheter pushed through the heart. In both methods, there is no need to perform an open surgery. The procedure can be completed under local anesthesia.

The TAVI method is primarily recommended for high risk patients who would otherwise not be able to undergo open surgery. It can also be used on patients who cannot have open surgery for other reasons. The TAVI method is proven to be effective on such patients in prolonging their lives and improving their clinical condition.
